Comments:
An unannounced renewal inspection was completed on 08/07/2019 from 10:00 A.M.-3:30 P.M. At the time of the inspection 43 children were in care with eleven staff, and one administrator present. Ten children records, six staff records, one board members record,two medications and required documentation, six injury reports, first aid kits, emergency supplies, playground, injury prevention plan, parent handbook, staff handbook, health inspection, fire inspection, and posted required information were reviewed. Children were observed during infant feeding, diapering, handwashing, one-on-one infant interactions, free choice activities, story time, music and movement,outdoor play, lunchtime, naptime, and afternoon play. Standard #: 22VAC40-185-130-B
Description: Based on review of children files, the center failed to ensure that additional immunizations were obtained once every six months for children under the age of two years. Evidence: 1.Child #2 last documented date of immunizations was 11/30/2018. Date of birth for Child #1 is 01/03/2018. 2.Child #10 last documented date of immunizations was 01/22/2019. Date of birth for Child #10 is 10/14/2018. 3. Staff verified the last documented date of immunization obtained, and date of birth of the children.

Plan of Correction: Administration will meet with the staff member responsible for children records and review the requirement for immunization updates for children.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that proper diapering procedures were followed. Evidence: 1. When changing an infants diaper, Staff #1 was observed disposing of a soiled diaper. Staff #1 turned to place the diaper in the foot operated waste can. Instead of using her foot to open the waste can lid she opened the lid with her hand and disposed of the diaper. 2. After changing the infant Staff #1 used a bleach solution to sanitize the diapering pad. Staff #1 did not first clean the area with soap and water. 2. Staff #1 verified that she did not use the foot pedal to open the waste can lid, and that she did not use soap and water before sanitizing the diaper pad.

Plan of Correction: Administration will meet with the staff and review the proper procedure for diapering.
