Comments:
The licensing inspector conducted an unannounced monitoring inspection that began at the center at 9:20am and concluded at the central office at 12:30pm with staff record reviews. There were twelve infants/toddlers in care: seven children in classroom one with two staff and five children in classroom two with two staff. The children were observed at free play, playing at the water table and playing on the playground. Diapering was also observed. One staff's record and five children's records were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical exam. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (children's records #2 and #3) did not contain documentation of a physical exam. Both children have been enrolled for more than 30 days. Child #2's date of enrollment was 05/21/2019 and child #3's date of enrollment was 06/19/2019.

Plan of Correction: The health coordinator will follow up with the children's parents to ensure a copy of the physical exams are obtained.

Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #4) did not contain documentation of viewing proof of the child's identity and age. Child's date of enrollment was 01/16/2019.

Plan of Correction: The family resource specialist will follow up with the child's parents to ensure proof of identity and age is obtained.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have all hazardous substances stored in a locked place. Evidence: 1. The inspector observed the wall cabinet in the bathroom adjacent to classroom #2. The lock on the cabinet door was broken. The cabinet contained glass cleaner, laundry detergent and disinfectant spray. The substances were inaccessible to the children, but were not stored in a locked place.

Plan of Correction: The lock will be replaced on this cabinet and staff will monitor to ensure all hazardous substances are stored in a locked place.

Standard #: 22VAC40-185-510-E
Description: Based on observation, the center failed to have a medication for a child in the original container. Evidence: 1. The inspector observed the medication for child #6 in classroom #2. The medication was not in the original container and did not have the child's name, dosage amount, or the time or times to be given.

Plan of Correction: The health coordinator contacted the child's parent and the original container will be brought to the center at pick-up time today.
