Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, August 20, 2019. The on-site inspection was from 10:35 a.m. to 3:05 p.m. On this date, there were 40 children present in four classrooms and in the care of eight staff members. There was one additional staff member available to assist if needed. There were 85 children enrolled in the center. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were listed on the supplemental page. There were six staff members' records and five children's records reviewed. Four classrooms were toured and observations were made in two of those rooms. There were six staff members and four children interviewed. The medication and medication paperwork were reviewed for two children. Lunch was observed to meet USDA guidelines. Some of items reviewed and observed during the inspection included: choice time, outdoor play, story time, lunch, infant care, diapering procedures, bathroom and hand washing procedures, nap time, updated health and fire inspections, updated emergency preparedness plan, required posted information, documentation of evacuation, shelter-in-place, and lockdown drills, water temperature and the arrival of school-age children. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of six staff records and confirmation from the director, one staff member did not obtain an updated tuberculosis (TB) screening within two years of the most recent screening. Evidence: 1. The record of Staff Member #2 showed that the most recent TB screening was obtained on 06/20/2018. The date of the previous TB screening was 01/11/2016. 2. The director confirmed that the TB screening was not obtained within two years of the most screening.

Plan of Correction: The director will ensure that from now on, she will have staff obtain TB screenings within two years of the most recent screening.

Standard #: 22VAC40-185-280-G
Description: Based on observation and confirmation with staff, a hazardous substance not in the original container was not labeled indicating the contents. Evidence: 1. The spray bottle containing bleach water and used for sanitizing the diaper changing pad in one classroom was not labeled indicating the contents of the bottle. 2. Staff Member #6 confirmed that the bottle was not labeled.

Plan of Correction: The bottle was labeled during the inspection.
