The licensing inspector conducted an unannounced monitoring inspection from 9:00am - 11:30am. There were eighteen preschool children in care with two staff. The children were observed playing in the centers, at circle time, playing on the playground and during lunch. Five children's records were reviewed. Standard #: 22VAC40-185-60-A Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. Two of five records reviewed (children's records #3 and #5) did not contain the following: work phone number and employment of each parent who has custody and previous child day care and/or school attended by the child. Plan of Correction: The teacher will contact the children's parents and obtain the missing information.
