An unannounced inspection was conducted on Thursday, July 18, 2019 in response to a self-reported incident that the department received on July 15, 2019. The inspector arrived at 9:55 AM and concluded the inspection at 10:45 AM. Interviews with staff were completed, playground resilient surfacing measurements were taken and documentation was reviewed. The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.
