22VAC40-185 ADMINISTRATION.

An unannounced monitoring inspection was conducted from 7:15 - 8:15 am with the Assistant Managers. There were nine children, ranging from five years to ten years of age, present with two staff supervising. The children were observed participating in a variety of activities including playing cards, reading, building and playing with gears. One medication was reviewed and in compliance. Two staff are Medication Administration Training (MAT) certified. The last emergency escape drill was conducted 7/22/19, and the last shelter-in-place drill was conducted 7/3/19 - please be sure to conduct an evacuation drill before the end of August. Standard #: 22VAC40-185-130-A Description: Based on a review of five children's files, the center did not obtain documentation that each child has received required immunizations prior to the child's first attendance. Evidence: The file for child #4 (date of first attendance: 8/12/19) and child #5 (date of first attendance: 8/12/19) did not have documentation of immunizations. Plan of Correction: Will obtain a copy of their immunizations and place it in their file.
Standard #: 22VAC40-185-70-A Description: Based on review of two staff files, and an interview with staff, not all required staff information was available on site. Evidence: Staff #1 and staff #2 did not have the name, address and telephone number of a person to be notified in an emergency. Plan of Correction: The information is available in their file at the Massad YMCA. A copy of the information will be made and kept on site at the school.
