An unannounced monitoring inspection was conducted between 12:00pm and 3:00pm. Upon arrival there were 52 children and 13 staff present. Children were observed during nap and infants were observed sleeping in cribs, playing on the floor and being held by staff. Three staff records were reviewed in addition to 2 medications.

Standard #: 22VAC40-185-560-F Description: Based on a observation and interview the center did not ensure all requirements were met when choosing to provided meals or snacks.
Evidence:
The center did not have a menu posted in the center. The director confirmed the menu is sent home to parents, however that day the children were served something that deviated from the menu.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The center began providing parents a calendar in advanced, but hadn't taken into consideration menu changes. A master calendar will be posted in the front lobby.
Standard #: 22VAC40-191-60-C-2 Description: Based on a record review the center did not ensure all staff had the required background checks.
Evidence:
The following staff did not have documentation of a central registry check: Staff #1(hired 06/17/19), Staff #3 (hired 05/06/19).Plan of Correction: The following corrective action was provided by the director during the exit interview:
The center sent for a criminal record check by mistake. Central Registry Checks will be sent out immediately.
