An unannounced monitoring inspection was conducted between 8:00am and 12:00pm. A sample of 6 children and 4 staff records were reviewed. Children were observed on the playground, circle time and playing with staff. Infants were observed sleeping in cribs, being fed and playing on the floor.

Standard #: 22VAC40-185-160-A Description: Based on a record review the center did not ensure each staff member had a negative tuberculosis screening no later than 21 days after employment.
Evidence:1. Staff #1 (hired 07/22/19) did not have documentation of a tuberculosis screening. The director confirmed the staff member did not have one and was sent that day to obtain it.Plan of Correction: The following corrective action was provided by the director during the exit interview:
Staff #1 was sent that day to obtain a tuberculosis screening.
Standard #: 22VAC40-185-70-A Description: Based on a record review the center did not ensure all staff had written information demonstrating their qualifications for their job positions.
Evidence: Staff #1 (hired 07/22/19) was listed in the file as a program leader. The director confirmed that was her job position. The center did not have documentation that Staff #1 met the qualifications of a program leader. The director confirmed that she had not seen a transcript of any other form of written information that confirms her qualifications.Plan of Correction: The following corrective action was provided by the director during the exit interview:
Staff #1 had qualifications listed on the resume, however a transcript was not received. The director will follow-up with staff #1 to obtain a copy of her qualifications for the job position as program leader
