An unannounced monitoring inspection was conducted on 9/3/19 from 12:00pm - 2:30pm. During the inspection there were 45 preschool aged children in care with 8 staff. Children were observed participating in various activities in the classrooms, and nap time. Records were reviewed for five children and five staff. Medication, injury reports, emergency procedures and emergency supplies were reviewed during the inspection. Licensing inspector also viewed the room to be used for the proposed modification during this visit. Areas of noncompliance are detailed in the violation notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-130-A Description: Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #1(in care) did not contain documentation of immunizations. 2. Staff confirmed the record for child #1 did not contain documentation of immunizations. Plan of Correction: The parent will bring immunization documentation for the record.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview it was determined the center did not ensure each child's record contained all of the required components. Evidence: 1. The children's records reviewed did not contain the addresses for the required emergency contacts. 2. Staff confirmed that the children's records reviewed did not contain the addresses for the emergency contacts. Plan of Correction: The school will research options to include emergency contact addresses in the enrollment paperwork.
Standard #: 22VAC40-185-550-M Description: Based on record review and interview it was determined the center did not ensure that the record of children's serious and minor injuries contained all of the required components. Evidence: 1. The injury report for child #6 dated 8/21/19 did not contain future action to prevent recurrence and date, time, and method of parental notification. 2. Staff confirmed this information was not on the injury report. Plan of Correction: Staff will add missing components to the injury report.
