Technical Assistance:
Technical assistance was offered regarding background checks and medications, and staff belongings.

Comments:
An unannounced monitoring inspection was conducted from 10:30am to 1:00pm. There 38 children with 7 staff members. The children were observed in circle time, music and outdoor activities. The children were also observed eating a morning snack. 5 children's and 5 staff records were reviewed. There were medications on site which were also reviewed. Violations:
Standard #: 22VAC40-185-350-E-4
Description: Based on observation, the center did not maintain a 1 staff to 10 children ration in the 3 years year classroom.
Evidence:
1. At 10:40am there were 21 3-4 year old children with 2 staff members, in the Shepherd's classroom.
2. The third staff member arrived in the room at 10:44am.
3. The classroom was over ratio for 4 minutes.

Plan of Correction: Additional staff will be placed to maintain ratio.

Standard #: 22VAC40-185-510-C
Description: Based on record review, the center did not obtain written authorization for long term prescription medication.
Evidence:
1. In the Angel's classroom, there were two long term medications for two children.
2. Child # 1 had an emergency medication. A written authorization from the physician was not obtained by the center for this medication.
3. Child # 2 had fever medication. The written authorization did not meet the requirements listed by the standards, and did not list the duration for the medication.

Plan of Correction: I will ask the parents to get the forms completed.

Standard #: 22VAC40-191-60-B
Description: Based on record review and staff interview, the center did not obtain a sworn disclosure statement (SDS) for each employee by the first day of employment.
Evidence:
1. Staff 1 and Staff # 3's first day of employment was 8/19/19.
2. A complete SDS was not obtained and maintained on file for both staff members.
3. Staff # 4's first day of employment was August 2015. A complete SDS was not obtained for Staff # 4 and was not available for review. Staff # 3 stated that Staff # 4's SDS may be in her record at the sister site.

Plan of Correction: Both staff will complete the form and it will be added to their records.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and staff interview, the center did not obtain fingerprint results for employees before the first day of employment.
Evidence:
1. Staff # 1's first day of employment was 8/19/19. Staff # 2 stated that Staff # 1 went to get fingerprinted on 8/5/19, and the results have not come in yet.
2. Staff # 3's first day of employment was 8/19/19. Staff # 2 stated that Staff # 3 went to get fingerprinted on 7/31/19. The results have not been obtained.

Plan of Correction: We will conduct a follow up and provide an update to the inspector.
