Comments:
An unannounced renewal inspection was completed on 9/12/2019. There were 20 children present with one staff supervising . Second staff arrived at 7:15 A.M. Five children's records were reviewed during the inspection. Three staff's records were reviewed on a previous date and were up to date. No medications at the site.

The inspector arrived at the school at 6:30 am and departed at 8 AM.

The children were observed at arrival, breakfast brought from home, free play with table toys and arts and craft.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of required staff information to be at the center, the center failed to ensure that name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.

Evidence:
1. The center did not have the required information at the center. It is at the main child care office.

Plan of Correction: The Amherst child care director will obtain all the required information and will make sure it is located at each site.

Standard #: 22VAC40-185-340-C
Description: Based on observation and interviews with staff, the center failed to ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.

Evidence:
1. Observed one staff person upon arrival into the program at 6:35 A.M. Staff # 1 stated that this is not the norm, staff had to be rearranged to cover additional sites. The school janitor is present but did not have direct means of communication with staff # 1. At 7 A.M. the school cafeteria ladies arrive and are in the same area as the morning program.

Plan of Correction: The site director will speak with the janitor to ensure that they have direct means of communication when there is only one staff present.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that all hands were washed before and after eating snack.

Evidence:
1. Observed four children that brought in breakfast from home, they did not wash hands with soap and water or use a disposal wipe before and after eating.

Plan of Correction: The staff will make sure that all hands are washed or a disposable wipe is used before and after.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that the tables were sanitized before and after eating.

Evidence:
1. Four children brought in food from home to eat at the morning program. The tables were not sanitized before and after.

Plan of Correction: The staff will sanitize tables when they arrive and will sanitize after the children are finished.
