The licensing inspector conducted an unannounced inspection on Monday, August 26, 2019, and received follow up documentation on Thursday, September 5, 2019 in response to a self-reported incident that the Department received on August 12, 2019 regarding inappropriate behavior guidance. The inspector was on site at the center from 10:20 am to 3:45 pm. Interviews with staff and children were completed and documentation was reviewed. The preponderance of the evidence gathered during the investigation validates the self-reported incident. Violations were cited. Standard #: 22VAC40-185-40-E Description: Based on staff & child interview and a review of documentation on 08/26/2019 and 09/05/2019, the center did not ensure that the center's activities comply with their own policies.
EVIDENCE:
1. Staff stated that Staff #1 was observed raising his voice at Child #1
2. Child #1 stated that Staff #1 "started yelling at me"
