Comments:
An unannounced monitoring visit and facility tour was conducted from 12:15 pm- 1:45 pm. At the time of inspectors' arrival there were 23 preschool aged children in care with 5 staff members and 1 administrator. Children were observed interacting with staff,nap, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was peeling paint on the wall in the two year old area. 2. There was exposed rust on the partitions in the girls restroom. 3. There were pedals missing on 7 of the bicycles on the outdoor "Merry Go Cycles". 4. There was exposed rust on the four swings outdoors.

Plan of Correction: All items will be repaired or replaced.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview it was determined that the center did not implement the two required shelter-in-place practice drills required. Evidence: 1. The last shelter-in-place practice drill on the log was dated 2017. 2. The Director confirmed that there was no documentation of shelter-in-place practice drills on the 2018 or 2019 logs.

Plan of Correction: Director will ensure that shelter-in-place practice drills are practiced and documented twice per year.

Standard #: 22VAC40-185-550-M
Description: Based on record review and interview it was determined the center did not ensure that the written record of children's serious and minor injuries contained all of the required components. Evidence: 1. The injury report for child #1 dated 6/25/19 did not contain the date, time and method of parental notification and one of the two required signatures. 2. The Director confirmed that the injury report for child #1 dated 6/25/19 did not contain all of the required information.

Plan of Correction: Staff were retrained on completing injury reports.
