Background checks - the sworn disclosure statement, search of the central registry (CPS check), and fingerprint check are valid for five years.
TB tests/screenings are valid for two years.

An unannounced renewal inspection was conducted with center director on September 18, 2019 to determine compliance with the licensing standards. There were 21 children present, ranging in age from 18 months to 4 years old, with three staff actively supervising. The children were watching a movie, eating lunch, and preparing for rest time. Lunch served today: salisbury steak, green beans, bread, applesauce, and milk. Two staff files and five children's files were reviewed - please see Supplemental Summary for details. There are no medications on site today; one medication goes home with the child each day. One staff is certified in MAT. The last health inspection was conducted 4/5/2019.

Standard #: 22VAC40-185-130-A Description: Based on review of five children's records, the facility failed to have documentation of an immunization record for each child before the child attended the center. Evidence: Child 1 (date of enrollment 4/2019) did not have documentation of an immunization record on file. Plan of Correction: Center director requested the immunization record from the parent during today's inspection.
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, the facility failed to have documentation of a physical for each child prior to the first day of attendance. Evidence: Child 1 (date of enrollment 4/2019) did not have documentation of a physical on file. Plan of Correction: Center director requested the child's physical during today's inspection.
Standard #: 22VAC40-185-160-C Description: Based on review of staff files, the facility failed to have documentation of a current tuberculosis test or screening for each staff within two years from the previous test. Evidence: Staff 3 did not have documentation of a repeat tuberculosis test or screening on file. Plan of Correction: Staff will get updated tuberculosis test or screening every two years.
Standard #: 22VAC40-185-350-E-3 Description: Based on observation and review of children's records, the facility failed to maintain the ratio of two year old children - one staff member for every eight children. Evidence: Upon LI arrival, there were ten children, ages two and three, present with one staff person. Plan of Correction: The two children that have not turned three returned to the two year old classroom.
