Comments:
An unannounced monitoring inspection was completed on 09/19/2019. There were 19 children, ages 3 years-4 years, 2 staff members providing direct care and supervision and one support staff member present during the inspection. The inspector reviewed 5 children's records and the on-site staff record requirements for 2 staff members during the inspection. The complete staff records will be review at the Corporate Office on a later date. The staff stated that medication is not currently administered. Children were observed in the following activities: during center/free choice play, participating in a learning art project, in large motor play outside, during story time and having lunch. The inspector reviewed the emergency supplies during the inspection. The licensing inspector discussed children's allergies with the staff. The inspector arrived for the inspection at 10:45 a.m. and departed at 1:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-330-B
Description: Based on observations, measurements and discussion with the staff, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that required a resilient surface.

Evidence:
1. The green slide measured 4 feet tall and required an 8 feet fall zone at the chute (in front of) of the slide. The fall zone at the chute of the slide measured 5 feet.
2. The swing set measured 6 feet 6 inches high and required a 13 feet fall zone in the front and behind of the swings. The fall zone in front of the swing set measured 11 feet.

Plan of Correction: The staff will contact administrative staff to have more mulch delivered.

Standard #: 22VAC40-185-540-C
Description: Based on observations and discussion with the staff, the facility failed to ensure that first aid kids shall contain the minimum items required by the standards.

Evidence: The first aid kit did not contain adhesive tape.

Plan of Correction: Site staff contacted administrative staff to request some during the inspection. The tape will be brought to the center.
