Comments:
An unannounced inspection was completed on 09/23/2019 from 9:50 A.M.-2:30 P.M. At the time of the inspection 36 children were in care with nine staff and one office administrator, and one administrator present. Five children records, five staff records, one medication and required documentation, two injury reports, health inspection, fire inspection, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during outdoor play time, transition to the classroom, snack time, handwashing, morning work cycles, departure of half day students, lunchtime, diapering, and naptime.. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In Classroom #2 there was a container of Clorox Disinfectant Wipes sitting on the half wall, and a bottle of Clorox Anywhere Disinfectant Spray on the changing table in the restroom.
2. In Classroom #4 there was a container of Clorox Disinfectant Wipes on the counter next to the microwave, and a bottle of Clorox Anywhere Disinfectant Spray setting on a cabinet in the restroom.
3. The warning labels stated "Caution", and "keep out of reach of children."
4. Staff verified that the Clorox Disinfectant Wipes, and the Clorox Anywhere Disinfectant Spray were not in a locked location.

Plan of Correction: Staff immediately placed materials into a locked location. Administration will review the requirements regarding hazardous materials.

Standard #: 22VAC40-185-280-G
Description: Based on observation, the center failed to label containers of hazardous substances that were not kept in an original container.

Evidence:
1. In Classroom #2 there was a spray bottle with a blue substance, that staff verified as Windex, that was not in the original container.
2. Staff verified that the spray bottle was not labeled with the name of the substance in the spray bottle.

Plan of Correction: Administration will review hazardous materials requirements with all staff.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that proper diapering procedures were followed.

Evidence:
1. The licensing inspector observed that during diaper changes that staff did not clean the diaper changing table with soap and water, or follow-up with sanitizing the surface between diaper changes. The licensing inspector observed staff wipe the surface with a baby wipe and then proceed to change the next child.
2. Staff verified that the proper procedure for cleaning and sanitizing the diaper changing table had not been followed.

Plan of Correction: Staff immediately started using the correct cleaning and sanitizing procedure. Administration will review the diaper changing policy with all staff.

Standard #: 22VAC40-185-520-A
Description: Based on observation of over the counter skin products, the center failed to ensure that all over the counter skins products shall not be kept beyond the expiration date of the product.

Evidence:
1. In Classroom #2 there was a bottle of Toys-R-Us Kids Sunscreen that expired 01/2016.
2. In Classroom # 2 there was a bottle of Babyganics Suncreeen that expired on 04/01/2019.
3. Staff verified that the sunscreens had expired.

Plan of Correction: Staff will return the expired medication to the parents. Administration will work on a policy to put in place to review over the counter skin products expiration dates.

Standard #: 22VAC40-185-520-B
Description: Based on observation of over the counter skin products, the center failed to ensure that sunscreen was labeled with the child's name.

Evidence:
1. In Classroom #2 in the cabinet there was a container of Toys-R-Us Kids Sunscreen that was not labeled with the child's name.
2. Staff verified that the sunscreen did not contain a label with the child's name.

Plan of Correction: Administration will review will all staff the requirements for all over the counter skin products be labeled with a child's name.
