Comments:
An unannounced monitoring inspection was conducted on September 25, 2019 to review licensing requirements. Three staff were present with ten children at the onset of inspection. Five children and four staff records were reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 5 children's records has a physical examination within a month of beginning attendance.

Evidence: The record for child #3 (start date December 18, 2017) did not have a physical. Assistant Administrator confirmed physical was missing from the record.

Plan of Correction: Parent has been contacted and physical will be brought in at pick up today.

Standard #: 22VAC40-185-290-3
Description: Based on observation, facility did not ensure electrical outlets have protective covers that are of a size that cannot be swallowed by children.

Evidence: In the preschool room, four outlets were not capped with a protective cover. Staff confirmed outlets were not covered and they were capped with covers during inspection.

Plan of Correction: Outlets were covered during inspection. Staff were reminded to keep outlets covered at all times.

Standard #: 22VAC40-185-500-C
Description: Based on observation and interview, facility did not ensure potty chairs are emptied promptly and cleaned and sanitized after each use.

Evidence: In the preschool class bathroom, a potty chair was observed with a dried brown substance in the bowl. Staff stated the potty chair had been used earlier that morning. The potty chair was cleaned and sanitized during inspection and the bowl was observed to be white after the cleaning. (Exhibit A)

Plan of Correction: Potty chairs will be cleaned immediately after every use. There will be a check done three times a day to make sure it is being done properly.
