An unannounced monitoring inspection was completed on September 19, 2019 from approximately 2:00 p.m. to 3:15p.m. Nine children were in care and three staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include outside play, indoor learning time, and nap time. Interviews and interactions were conducted and medications were reviewed. Five children?s records and five staff records were reviewed. The directors/owners were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-510-E Description: Based on observation and interview, the licensee failed to ensure that medication is labeled with the dosage amount and the time or times to be given.
Evidence: 1. A prescription epi-pen was observed in a plastic box labeled with Child #6's name. The epi-pen was not in the original, labeled container and lacked the dosage amount and time or times to be given. 2. In an interview, Administrator #2 acknowledged the epi-pen is not labeled as required.Plan of Correction: A director/owner will obtain the label from the child's parent.
