An unannounced monitoring inspection was completed on September 23, 2019 from approximately 10:00 a.m. to 12:00 p.m. Fifty-six children were in care and twelve staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include structured learning time, outside play, and chapel time. Interviews and interactions were conducted and medications were reviewed. Five children?s records and five staff records were reviewed. The director were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-70-A Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of five staff.
Evidence: 1. Staff #5's record, hired on July 1, 2019, as a program director, lacked documentation of education required for the job position. A resume in the record indicated that Staff #5 possesses the required education but there was no documentation in the record of such education. 2. Administrator #1 acknowledged this documentation was not present in the record.Plan of Correction: The director will obtain education documentation from the staff.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records and interview, the licensee failed to obtain a fingerprint results prior to employment for one of five staff.
Evidence: 1. Staff #5's record, hired on July 1, 2019, contained fingerprint results dated July 3, 2019 and therefore not obtained prior to employment. 2. Administrator #1 acknowledged this staff began employment prior to obtaining the fingerprint results.Plan of Correction: This cannot be corrected. These fingerprint results were obtained late prior to the new director's hire. The new director is aware of the fingerprint requirements.
