An unannounced monitoring inspection was conducted from 9:45 am - 11:30 am with the office administrator and owner. There were 23 children, all four years of age, supervised by four staff. Children were observed completing table work, using the restroom, dressing up and practicing a fire drill. Five children's records and five staff records were reviewed. There were no medications to review. The playground was not inspected due to inclement weather. The emergency drill log was observed and in compliance. The last health inspection was conducted: 8/6/19 and the last fire inspection was conducted: 8/6/19. Standard #: 22VAC40-185-160-C Description: Based on a review of five staff records, it was determined that not all staff had repeat tuberculosis (TB) tests/screenings every two years. Evidence: The last TB test/screening for staff #2 is dated: 8/22/17 and for staff #3 is dated: 8/23/17. Plan of Correction: Staff will obtain a new TB test screening this week.
Standard #: 22VAC40-185-60-A Description: Based on a review of five children's records, it was determined that not all records had all required information. Evidence: The file for child #1 and child #2 did not have phone numbers for two emergency contacts. Plan of Correction: Will ask the parents for the missing information.
Standard #: 22VAC40-185-80-A Description: Based on a review of the written attendance record, and staff interviews, the center did not ensure all children were included on the written attendance record. Evidence: the written attendance record for room 1, 3 and 4 did not include all children in attendance. Plan of Correction: Will ensure all children are signed in as they arrive on the attendance sheets.
