63.2(17) License & Registration Procedures

Comments:
An unannounced inspection was completed on 09/6/2019 from 9:45 A.M.-1:45 P.M. At the time of the inspection 78 children were in care with nine staff, one administrative assistant, and one administrator present. Seven children records, seven staff records, four medications and required paperwork, six injury reports, emergency plan, first aid kits, emergency supplies, playground, playground safety plan, and posted required information were reviewed. Children were observed during morning work cycle which included sensory activities, literacy, geography lessons, science activities, puzzles, fine motor activities, handwashing, birthday celebration, lunchtime, departure of half day students, and preparation for naptime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-520-A
Description: Based on review of over-the-counter skin products, the center failed to ensure that skin products were not kept beyond the expiration date.

Evidence:
There was a bottle California Baby Sunscreen in the covered tote used for storage of over-the-counter skin products that had expired in January 2018.

Plan of Correction: Administration will remove the sunscreen and dispose of it immediately.

Standard #: 22VAC40-185-520-D
Description: Based on review of over-the-counter skin products, the center failed to ensure that all containers were labeled with the child's name.

Evidence:
There were two bottles of Off Insect Repellant in the covered tote used for storage of over-the-counter skin products that were not labeled with the child's name.

Plan of Correction: Administration will review the parent authorization forms to determine what child the insect repellent belong to, and label with the child's name.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of staff records, the center failed to ensure that all staff had a completed fingerprint background check on file prior to the first day of employment.

Evidence:
1. Staff #3 fingerprint background check was dated 08/26/2019. Start day of Staff #3 was 08/14/2019.
2. Staff verified the start date for Staff #3, and the date of completed fingerprint background check.

Plan of Correction: Administration was unaware that the fingerprint must be received before the first day of employment, she was under the impression they had 30 days upon hire to have completed background checks on file. In the future all staff will have a completed fingerprint on file before beginning employment.
