Technical Assistance:
Notarized religious exemption from immunizations was discussed with the program director.
Children with emergency medications was discussed.

Measurements were taken of a possible addition to the playground space by moving the fencing to the parking lot curb. This would allow for a total of fourteen children to be on the playground. It was discussed that moving the side fencing back another 6 feet, adding a 6 x 6 foot addition, would allow for another child (a total of 10) to be on the playground at one time.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 11:50 AM. Seven children were in care with two staff. Morning activities, mediation snack service and lunch service and outdoor play were observed. Two staff records and a sample size of five children's records were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported.
The playground fencing along the side has been moved. Measurements were taken and the additional space will allow for nine children to be on the playground at any one time.

Description: Based upon review of five children's records, the facility has not ensured that each child's record includes the complete street address of the two designated persons to be contacted in an emergency if a parent cannot be reached.
Evidence:
The record provided for child 4 did not have the street addresses of the two designated persons to be contacted if a parent cannot be reached.

Plan of Correction: The facility responded with the following:
Each child's record will include a complete street address for the two designated emergency contacts.

Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked locations using a safe locking method that prevents access by the children.
Evidence:
1. There was an unattended spray bottle identified by staff 1 as containing a bleach water mixture on the floor beneath the staff table in the classroom.
2. Staff 1 verified that the spray bottle was not in a locked location.

Plan of Correction: The facility responded with the following:
The sanitizing spray will be kept in the locked closet unless being directly supervised by staff person.

Description: Based upon observation and staff interview, the facility has not ensured that any substitute container used is clearly labeled as to the contents.
Evidence:
1. There was a spray bottle of fluid used to clean the tables.
2. Staff 1 verified that the contents were a bleach water sanitizing mix and that the spray bottle was not labeled as to the contents.

Plan of Correction: The facility responded with the following:
Any substitute containers will be clearly labeled as to the contents of the containers.
