An unannounced monitoring inspection was conducted with center staff. There were 25 children present, ages two through five, with 4 staff actively supervising. The children were playing outdoors, getting water, and settling down for rest time. Five children's files and five staff files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 2/6/2019. The last fire inspection was conducted 8/21/2019. Evacuation drills must be practiced monthly and shelter in place drills must be practiced twice a year. There are no medications on site today. Standard #: 22VAC40-185-70-A Description: Based on review of five staff files, the facility failed to have all information documented in each staff's file. Evidence: Staff 1 had two phone references, but they were not signed by the person making the call. Staff 3 did not have the signature of the person who obtained one phone reference. Staff 3 did not have written information to demonstrate that the individual possesses the education and experience required by the job position. Plan of Correction: Phone references will be signed by the person obtaining the reference. All staff will be asked to submit documentation of qualifications for the position.
Standard #: 22VAC40-185-190-A-4 Description: Based on review of five staff files and staff interview, the facility does not have a qualified director. Evidence: Staff 5 (promoted to the position of director 7/2019) has documentation of two years programmatic experience with one year in supervisory capacity, but does not have one of the required educational backgrounds for the position. Standard #: 22VAC40-185-450-A Description: Based on observation, the facility did not ensure that each child had a top and bottom cover for children resting on mats during the designated rest period. Evidence: Seven children did not have a top cover or bottom cover on their mat during rest; six children did not have a top cover on their mat during rest. Plan of Correction: Not available online. Standard #: 22VAC40-185-550-E Description: Based on review of the emergency drill log, the facility failed to have documentation of an evacuation drill each month. Evidence: there was no documented drill for August 2019 and September 2019. Plan of Correction: Evacuation drills will be conducted monthly and documented each month.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of five staff files, there was no documentation to support that all staff had a search of the central registry (CPS check) within 30 days of employment. Evidence: there was no documentation that Staff 1 (date of employment 10/2016) had a completed search of the central registry check. Plan of Correction: Center director sent a CPS check for Staff 1 on 10/4/2019.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of five staff files, the facility failed to have documentation of an original fingerprint check for each staff. Evidence: the fingerprint check on file for Staff 3 was from another child care facility. The center did not conduct a fingerprint check; fingerprints are not transferable. Plan of Correction: Staff 3 has an appointment to obtain original fingerprint check for the facility file.
