Comments:
An unannounced monitoring inspection was conducted from 4:00 - 5:00 pm. There were 33 children, ranging in age from five years to ten years of age, present with three staff supervising. The children were observed playing games, building with blocks and legos, completing homework and eating snack. Snack served today was cucumbers with ranch dressing, crackers and water. Four children's records were reviewed and in compliance. Two staff records were reviewed offsite at the Massad central office. The last emergency escape drill was conducted 9/20/19 and the last shelter-in-place drill was conducted 9/25/19. One medication and authorization form was reviewed and in compliance. Description: Based on observation, and staff interview, the facility did not have children wash hands with soap and running water or disposable wipes after eating snack. Evidence: children did not wash their hands or use disposable wipes after eating afternoon snack.

Plan of Correction: Will ensure children's hands are washed before and after snack.
