An unannounced monitoring inspection was conducted from 7:00 -8:30 am with staff. There were 10 children in care, ranging from five years to ten years of age, at the beginning of the inspection supervised by four staff. Children were observed having free play at tables and playing a group game of silent speed ball. The emergency drill log was reviewed and in compliance. The first aid kit was reviewed. Five children's files were reviewed and in compliance. Four staff files were reviewed offsite at the Massad central office on 9/17/19. Standard #: 22VAC40-185-80-A Description: Based on review of the written attendance record, the facility did not ensure all children were included on the written attendance record.
Evidence: The written attendance record for the month of October did not have attendance marked for any days in October.Plan of Correction: We will ensure all children are included on the written attendance record.
Standard #: 22VAC40-185-240-D-1 Description: Based on review of documentation and staff interview, the facility has agreed to administer prescribed medications but did not have a staff member with current Medication Administration Training (MAT) certification. Evidence: Staff #1's MAT certification expired 9/17/16 and Staff #2's MAT certification expired 5/7/16. Plan of Correction: Staff are signed up to take MAT in October.
Standard #: 22VAC40-185-540-C Description: Based on observation, the facility did not have all of the required items in the First Aid Kit. Evidence: The First Aid Kit did not contain tweezers, two triangular bandages, or a thermometer. Plan of Correction: We will obtain the missing items.
