Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, October 1, 2019 to determine the center?s compliance with licensing standards. The inspection was initiated at 02:30pm and concluded at approximately 6:20pm. The center's assistant director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 165 children. The children and staff were observed engaged in a variety of activities while the inspector was on site. Some of the children were observed during free play, doing homework, eating snack, engaged in teacher-led group activities, and playing outside. Transitions were observed throughout the inspection. The staff were observed having positive interactions with the children. All areas of the center including classrooms, hallways, bathrooms, and outdoor play areas were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were observed and found to be in compliance. Ten children's records, five children?s medications and authorizations, and ten staff records were reviewed. The menu was posted and today's afternoon snack consisted of a choice of 2: fresh fruit, tortilla chips and salsa, yogurt, WG Teddy Grahams, and WOW butter. The most recent evacuation drill was documented on 09/19/2019. The most recent shelter-in-place drills were documented on 9/25/2019 and 08/07/2019.

Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of ten staff records and interview on 10/01/2019, the center did not ensure that one staff shall submit documentation of a negative tuberculosis screening within the required time frame.

Evidence: 1) The tuberculosis screening in the record for Staff #8, hired on 05/11/2019, was dated 06/03/2019. 2) During interview, a member of management reported the center did not have other documentation of Staff #8's tuberculosis screening prior to 06/03/2019. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Plan of Correction: Per the Center: "Staff #8 participated in a pre-summer training on Saturday, May 11, 2019. Staff #8 was paid for the time as training hours. Staff #8 had neither started working with any children nor had completed our required new hire orientation. We were counseled by our licensing inspector that if the staff member is paid in any capacity the tuberculous screening must be completed at the time of the first paid day and not when the staff member actually starts with the program. We have added the tuberculous screening form to our new hire paperwork to complete at the time of hire in place of at the time of orientation."

Description: Based on a review of documentation on 10/01/2019, the center did not maintain a record of the dates of all practice drills for one year.

Evidence: 1) The licensing inspector reviewed the center's practice drill log. There was no drill documented for June 2019. 2) A center administrator reported an evacuation drill was conducted on June 1, 2019, but it was not documented in the practice drill log. The center shall maintain a record of the dates of the practice drills for one year.

Plan of Correction: Per the Center: "Site administration completed an additional training drill as part of summer training and preparation. As a supplemental drill, this drill was not logged on the main form. Site administration have been retrained to log all drills on the appropriate forms. For 2019 the program is on track to complete 12 practice fire drills with students, on top of supplemental training drills. The Village at Wyndham takes drill and emergency preparation very seriously and site administration will continue to be attentive to this through the monthly quality assurance process."
