Comments:
An unannounced monitoring inspection was completed on 10/10/19. There were 39 children present in four classes with nine staff directly supervising and three additional staff present to assist as needed. Five children's records and five staff records were reviewed. Four medications for four children in care were reviewed.

The inspector arrived at the center at 9:00 am and departed at 12:30 pm.

The children were observed during breakfast, during outside times, during structured story times, and during music/movement times. All infant activities were also observed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all required information was documented in each child's record.

Evidence:
1. The home address and work phone number for the father of child #2 was not documented. The information in the child's record noted that the father did not live with the child and there was a work place documented for the father.

Plan of Correction: Staff #1 will have the parent document this information.

Standard #: 22VAC40-185-420-E-1
Description: Based on interview, the center failed to ensure that for each infant, there was a daily record of activities for the infant to include amount of food consumed and the time, description and time of bowel movements, time spent on their stomachs, amount of time they slept, and developmental milestones.

Evidence:
1. The inspector asked the infant staff for the daily records for the infants. Staff #6 stated that they were not being done as the parents did not request them. The staff person stated that the records had not been done for several weeks.

Plan of Correction: Staff will make sure the daily records are completed for each infant.

Standard #: 22VAC40-185-510-D
Description: Based on observation and interview, the center failed to ensure that a medication authorization was available to staff.

Evidence:
1. An emergency medication (Epipen) was observed for child #3 for a food allergy. Child #3 is three years old. The medication had been at the center since the child enrolled in August 2019. There was no written authorization at the center from the parent or from the physician for this medication.

Plan of Correction: This was corrected during the inspection. Staff #1 called the child's physician office who had the "Individual Health Plan" form completed by the parent and by the physician. The form was completed September 3, 2019 by the parent and completed/dated by the physician the following week. Staff #1 obtained a copy from the physician's office for the child's record.
