24 hour lead teacher training process and requirements.

Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, group time, art, activities, personal hygiene, lunch time, and outdoor time were observed. Children were observed; and staff and children files and other documentation were reviewed. Eight (8) children, aged 16 months-5 years, were found to be in care with 2 staff, and administrative staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 10:10 am and concluded at 12 noon.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children files the facility failed to maintain compliance with this standard as required relating to emergency contact information.

Evidence:
Child record #3 did not have a phone number for the emergency contact. There was not a second emergency contact listed.
Child record #4 did not have the phone number and address for the second emergency contact.
Child record #5 did not have addresses for either of the 2 emergency contacts.

Plan of Correction: This information will immediately be obtained and recorded in the files. Files will be reviewed for compliance and corrected or updated as needed.

Standard #: 22VAC40-185-500-B
Description: Based on review of the diapering area the facility failed to maintain compliance with this standard as required relating to a non-absorbent diapering surface.

Evidence:
The diaper changing pad in the infant area had a few tears in the covering, which did not allow for a completely non-absorbent surface as required.

Plan of Correction: This will be replaced with a new one for compliance. Diapering pads will be viewed for compliance and replaced as needed in the future.
