Comments:
A mandated monitoring inspection was conducted today. The inspector arrived at 10:55 AM and departed at 4:05 PM. One hundred five children were in care. Morning activities, outdoor play, lunch service and preparation for nap time were observed. A sample size of ten children's records and ten staff records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the administrators prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based upon review of ten staff records and staff interview, the facility has not ensured that when staff are hired, results of a tuberculosis screening are obtained within 21 days of employ and the screening can be not older than one year before employment.
Evidence:
1. The record provided for staff 9 indicated a hire date of 8/26/2019. The tuberculosis screening on record was dated 12/1/2017 which is not within one year prior to hire.
2. Staff 3 verified that the tuberculosis screening for staff 9 was not obtained within one year prior to hire.

Plan of Correction: The facility responded with the following:
We will ensure that tuberculosis screenings will be obtained by newly hired staff as required.

Standard #: 22VAC40-185-510-A
Description: Based upon review of medication stored for emergency administration to children in care and review of records, the facility has note ensured that center's written medication policies are followed.
Evidence:
1. The facility's medication administration policies require written parent authorization for the administration of medication.
a. There is no written parent authorization for child 16 to be administered the albuterol prescribed and stored for emergency administration to child 16..
b. There is no written parent authorization for child 20 to be administered the epi-pen prescribed and stored for emergency administration to child 20.
2. The allergy management plans for child 11, child 13 and child 19 indicate the need for the administration of an antihistamine prior to epinephrine. The facility does not have the prescribed antihistamine for these three children or documentation from the parents regarding the antihistamines.
3. Staff 3 verified the above information.

Plan of Correction: The facility responded with the following:
Parents will be contacted to review allergy management plans, required medication and parent authorizations.

Standard #: 22VAC40-185-550-L
Description: Based upon review of injury records for September and October 2019 and staff interview, the facility has not ensured that parents are notified by the end of the day for minor children's injuries.
Evidence:
1. An injury record for child 3 documents that child 3 was injured on 9/20/19. The parent signed the record on 10/9/19. There was no documentation that the parent was notified the same day of the injury.
2. 1. An injury record for child 4 documents that child 4 was injured on 10/3/2019. The parent signed the record on 10/8/19. There was no documentation that the parent was notified the same day of the injury.
3. Staff 3 verified that the records did not include documentation that the parents were notified the same days as the injuries.

Plan of Correction: The facility responded with the following:
The written injury reports will be adapted to include a space for documentation of same day parent notification of injuries.
