An unannounced complaint investigation was initiated on September 16, 2019 in response to allegations received by the Division regarding ratio in the infant room, field trips to the pool, children's files, outdoor time, and food policies. The most recent parent handbook was provided on 10/7/2019. Based on staff interviews and documentation reviewed, the preponderance of evidence gathered during the investigation does not support the allegations of ratio in the infant room, children's files, food policies, or outdoor time. The allegations regarding field trips to the pool and pick up procedures were determined to be valid. Standard #: 22VAC40-185-40-E Complaint related: Yes Description: Based on review of the parent handbook, review of children's files, and staff interview, the facility failed to follow their own policy on departure of children, including persons authorized to pick up the children. Policy states "will not allow children to be released to person not authorized by a parent/guardian" and "the center must be notified in writing if someone other than the parent/guardian or authorized person is picking up your child." Evidence: Staff 4 stated that Child 1, Child 2, Child 3, Child 6, and Child 7 have been released to a person who was not documented as an authorized pick up person in the child's file. The center was given verbal authorization, but did not have anything in writing to approve the release of the children. Plan of Correction: Children will only be released to persons who the parent has authorized in writing to be released.
Standard #: 22VAC40-185-460-B Complaint related: Yes Description: Based on review of staff records and staff interview, the facility failed to have documentation that a certified lifeguard with a current certificate was on duty supervising the children participating in swimming activities. Evidence: Staff 1, Staff 2, and Staff 3 were the staff present on field trips to the swimming pool. None of the staff have documentation of current lifeguard certification.
