An unannounced renewal study was conducted with the center director on October 17, 2019 to determine compliance with the licensing standards. There were 24 children, ranging in age from 2 months to 4 years, present with 5 staff actively supervising. The children were having lunch and preparing for rest. Lunch served today: bologna sandwich, tater tots, pineapple, and milk. The last fire inspection was conducted 4/25/2019. The last health inspection was conducted 11/29/2018. Standard #: 22VAC40-185-160-A Description: Based on review of five staff files, the facility failed to have documentation of a tuberculosis test or screening within 21 days of employment. Evidence: Staff A (date of hire 6/4/2019) did not have documentation of a tuberculosis test or screening on file. Plan of Correction: Staff A had the test done on 7/30/2019, but does not have documentation of the test. The physician will send the information via fax tomorrow.
Standard #: 22VAC40-185-500-A Description: Based on observation, the facility failed to have staff wash their hands with soap and running water after a diaper change. Evidence: Staff A changed an toddler's diaper and did not wash her hands with soap and running water. Plan of Correction: All staff will be reminded to wash hands after changing diapers.
Standard #: 22VAC40-185-550-D Description: Based on staff interview and documentation of emergency drills, the facility failed to conduct two shelter in place drills each year. Evidence: the last documented shelter in place drill was 9/2018. Plan of Correction: Center director will conduct a shelter in place drill this month.
