An unannounced monitoring inspection was conducted from 4:00 - 5:30 pm with the Assistant Manager. There were 36 children, ranging in age from five years to 11 years of age, present with two staff supervising. The children were observed completing homework, reading, washing their hands and going outside to play. Five children's records were reviewed. Three staff records were reviewed offsite at the Ron Rosner central office on 10/9/19. The last emergency escape drill was conducted 10/4/19, the last shelter-in-place drill was conducted 10/4/19 and the last lockdown drill was conducted 10/18/19. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, it was determined that not all records contained all required information. Evidence: child #4 and child #5 did not have the addresses for two emergency contacts in their record. Plan of Correction: Will ask parents to provide the missing information.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation, the facility failed to maintain the required ratio of one staff for every 18 children, ages five years to eight years of age. Evidence: 1. When the LI arrived, one staff left the group to open the door, leaving one staff member alone with 36 school age children for less than two minutes. 2. Staff took 11 children to the bathroom leaving one staff alone with 25 school age children for approximately 5-10 minutes. The youngest child in attendance today is five years old. Plan of Correction: Will ensure that ratios are maintained, including by taking all children to answer the door if not enough staff are present.
