Technical Assistance:
Creating a tracking system to notify all staff regarding children whose lunches would need to be supplemented or children who did not have water bottles was discussed with staff 1 and 2.

Comments:
An unannounced, mandated monitoring inspection was conducted today. Morning registration, check-in and activities were observed. Children arrive, have activities and eat morning snack before leaving on the field trip where they eat lunch. Forty-one children were in care. Five staff were on duty. Medication procedures were reviewed. No children's injuries were reported thus far this school year.
An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based upon observation and staff interview, the facility has not ensured that all areas of the center are maintained to be safe.
Evidence:
1. There is a two foot long strip of vinyl cove base at the bottom of one column in the middle of the teen room that has pulled away from the column creating a tripping hazard. Children were observed in the teen room this morning.
2. Staff 1 verified that the cove base needed to be glued back into place.

Plan of Correction: The facility responded with the following:
YMCA Property Director was notified to repair the piece of vinyl at the base of the column.

Standard #: 22VAC40-185-340-F
Description: Based upon observation and staff interview, the facility failed to ensure that children are in sight and sound supervision of staff.
Evidence:
1. Child 1, five years old, was signed in at the registration desk by her parent. Four additional children were being signed in at the same time. Staff 4 was the staff designated to accompany the children from the registration desk to the teen center. Staff 1 was manning the registration desk. Staff 4 took the group of children down the hallway. Staff 1 was registering children. Child 1, who was eating her breakfast, walked away from the registration desk and sat at a table behind and to the left of staff 1. Staff were unaware that child 1 was sitting by herself at the table until notified by the inspector approximately five minutes later.
2. Staff 2 verified that she did not know that staff 4 did not take child 1 with the group walking to the teen center.

Plan of Correction: The facility responded with the following:
Check in operational procedures will be reviewed. To ensure this doesn't occur again, children will remain in a designated area in front of parent table before being escorted to the teen center. Breakfast will be in the Teen Center.
