Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 7, 2019 from 2:50 p.m. through 4:40 p.m. There were thirty-one children in the care of the provider and three staff. Five children's and two staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice.

Areas observed were: arrival, educational centers, hand washing/bathroom procedures, snack, and inside free play in the gymnasium. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure one child was in actual sight and sound supervision.

Evidence: 1. One child, was observed leaving the cafeteria at approximately 3:53 p.m. 2. At approximately, 3:55 p.m. the child returned to the cafeteria. 3. Staff were unaware the child had left the cafeteria during supervised group time.

Plan of Correction: Held a staff meeting immediately following inspection to discuss violations. Reviewed how to actively be engaged, but still have sight/sound at all times. Moving forward, when my Supervisor does a site visit, I have asked her to try and remove students from groups without staff knowing. If she is successful, disciplinary corrective action will be taken with staff.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to have children wash hands immediately before sitting down to eat snack.

Evidence: 1. Children and staff were observed leaving the tables they were using to play games and going to the bathroom to wash hands. 2. The children were observed returning to the cafeteria and sitting down sat down for snack to tables that had not been sanitized.

Plan of Correction: Spoke with Licensing Rep to ensure staff understood the order of sanitizing and washing hands. After washing hands, students must return to a sanitized table in order to keep washing of hands valid.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review, the center failed to document evacuation drills two consecutive months.

Evidence: 1. Evacuation drills were reviewed. 2. There was not documentation of the center conducting evacuation drills for the months of August and September of 2019. 3. The director confirmed they had not been conducted.

Plan of Correction: Created a record log for Fire and Shelter in Place Drills for Oct-June.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to sanitize tables before serving snack to the children in care.

Evidence: 1.Children were observed playing games at tables in the cafeteria. 2. The center served snack to the children before sanitizing the tables. 3. Staff confirmed they did not sanitized tables immediately before serving snack to the children in care.

Plan of Correction: Spoke with Licensing Rep to figure out a plan that best suits our program for sanitizing tables before and after snack. We now have 3 separate tables labeled just for snack. They are sanitized before students enter program and not used again until children return from restroom after washing hands. After snack, staff will walk around with trash can and handout baby wipes to have students clean hands and then throw away. Lastly, students will move on to next activity while staff sanitize snack table.
