63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 10/22/2019 from 10:15 am - 1:41 pm. At the time of the tour there were 97 children in care with 17 teaching staff and four administrative staff. The children ranged in age from infant - five years. Children were observed during late morning program time which included outdoor play. Nap was also reviewed.
A sample of eight children's records and six staff records were reviewed.
Violations were observed in three parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, the center failed to ensure that documentation of the tuberculosis screening shall have been completed within 12 months prior to employment.

Evidence:
1. Staff 1 has a hire date of 08/26/2019. The TB screening on file was completed on 03/16/2018.
2. Staff 2 has a hire date of 08/26/2019. The TB screening on file was completed on 05/14/2018.

Plan of Correction: Administration informed staff 1 & 2 that TB tests needed to be updated by 10/31/2019. Scanned copies of updated TB tests will be sent over as soon as received.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
The record for child 1 lacked documentation of a work phone number for one parent.

Plan of Correction: Administration had the child's parents update the file to include a work phone number.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center failed to ensure that the written record of children's injuries contained all of the required elements.

Evidence:
1. Ten of 30 injury reports reviewed lacked documentation of the time the parent was notified of the injury.

Plan of Correction: Re-trained all teachers on proper documentation for injury reports. The administrative coordinator will also begin reviewing filed copies of injury reports to ensure accuracy on a daily basis.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the center failed to ensure that no child shall be allowed to drink while walking around.

Evidence:
A child was observed drinking for a water bottle while walking with her class to the outdoor play area.

Plan of Correction: Teachers were re-trained on ensuring children do not drink their water bottles while they are standing.
