An unannounced inspection was conducted on October 25, 2019. At the time of entrance there were 14 children present with 3 staff members. The sample size of records reviewed consisted of 5 children's records and 4 staff records. Children were observed playing in multiple activity centers to include art projects, blocks, home-living and science activities. one medication were reviewed. The inspection began at approximately 9:30 am and ended at approximately 10:45 am. The violations from the previous inspection were reviewed and corrected.
