Comments:
An unannounced monitoring inspection was conducted on 10/30/19 from 2:15pm - 3:30pm. During the inspection there were 48 children ages four years old to ten years old in care with three staff. Children were observed participating in various activities in the cafeteria, doing homework and eating dinner/snack. Records were reviewed for five children while at the facility. The records for three staff were reviewed on 10/31/19 at the central office. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-350-E-5
Description: Based on on observation and interviews, it was determined that the licensee did not ensure that for children, from the age of eligibility to attend public school through eight years, one staff member for every 18 children is maintained at all times children are in care.

Evidence:
1. Staff #2 and staff #3 were working with a group of 38 school age children in the cafeteria. The children present were ages five to ten years old, so the facility was not maintaining the required 1 staff for every 18 children.
2. Staff #1 (Site Director) confirmed that the facility did not maintain the required staff-to-child ratio during the inspection.

Plan of Correction: The facility responded: We are in the process of hiring more staff, but will move staff around to ensure the staff-to child ratios are maintained at all times.
