Comments:
A Subsidy Health and Safety inspection was conducted on Wednesday, October 23, 2019 to review supplemental health and safety requirements.The inspection was initiated at 1:30 pm and concluded at approximately 5:30 pm. The center?s director and owner were present and assisted the inspector throughout this inspection. The census for today?s inspection consisted of 26 children in the direct care of five staff members. Upon the inspector?s arrival, the preschool children were observed in their respective classrooms resting and napping. The children were later observed as they were waking up, taking bathroom breaks, washing hands, and eating an afternoon snack. The school-age children later arrived at the center and were observed during transitions. A group of children were observed playing outside and interacting with peers and staff. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. All areas of the center including the classrooms, hallways, kitchen, bathrooms, and playgrounds were inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. Per the director, there are no medications on site at this time. Five children's records and five staff records were reviewed during today's inspection. The most recent evacuation drill was documented on 09/09/2019. The most recent lockdown drill was documented on 06/12/2019. And two shelter-in-place drills were documented on 06/17/2019 and 09/17/2019.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-650-E
Description: Based on a review of the center's policies and interviews on 10/23/2019, the vendor did not ensure that they shall develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence: 1) There was no written policy and procedure regarding consistent staff in each group of children available for viewing during the inspection. 2) A member of management confirmed that a written policy and procedure regarding consistent staff in each group of children had not been developed in writing.

Plan of Correction: Per the Vendor: The missing policy will be added to the center's policies and memo reviewed.

Description: Based on a review of documentation and interview on 10/23/2019, the vendor did not ensure the emergency preparedness plan contained continuity of operations procedures to ensure the essential functions are maintained during an emergency.

Evidence: 1) During the inspection, the licensing inspector reviewed the emergency preparedness plan. The plan did not contain a written plan on how the center will ensure continuous operation procedures are in place to ensure essential functions are maintained during an emergency. 2) During interview, a member of management reported the information had not been added to the emergency preparedness plan to date.

Plan of Correction: Per the Vendor: The procedure will be added to the emergency preparedness plan and memo reviewed.
