Comments:
An unannounced monitoring inspection was conducted on Wednesday, October 23, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 1:30 pm and concluded at approximately 5:30 pm. The center?s director and owner were present and assisted the inspector throughout this inspection. The census for today?s inspection consisted of 26 children in the direct care of five staff members. Upon the inspector?s arrival, the preschool children were observed in their respective classrooms resting and napping. The children were later observed as they were waking up, taking bathroom breaks, washing hands, and eating an afternoon snack. The school-age children later arrived at the center and were observed during transitions. A group of children were observed playing outside and interacting with peers and staff. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. All areas of the center including the classrooms, hallways, kitchen, bathrooms, and playgrounds were inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. Per the director, there are no medications on site at this time. Five children's records and five staff records were reviewed during today's inspection. The most recent evacuation drill was documented on 09/09/2019. The most recent lockdown drill was documented on 06/12/2019. Two shelter-in-place drills were documented on 06/17/2019 and 09/17/2019.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records and interview on 10/23/2019, the center did not obtain documentation that one child had received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: 1) The immunization documentation in the record for Child #4, enrolled on 6/17/2019, was dated 6/21/2019. 2) During interview, a member of management reported the center did not have prior documentation of Child #4's immunization documentation before the child attended the center.

Plan of Correction: Per the Center: Already corrected. Moving forward, children will not be enrolled until the immunizations are in. Management will develop a checklist for children's registration to ensure the information is submitted on time.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records and interview on 10/23/2019, the center did not ensure one child had a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence: 1) The physical examination in the record for Child #4, enrolled on 06/17/2019, was dated 10/02/2019. 2) During interview, a member of management reported the center did not have prior documentation of Child #4's physical examination.

Plan of Correction: Per the Center: Already corrected. Moving forward, parents will receive a letter to inform them the child cannot return (partial termination) until the needed documentation is returned. Management will develop a checklist for children's registration to ensure the information is submitted on time.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records on 10/23/2019, the center did not ensure that three children's records contained the required information.

Evidence: 1) The record for Child #2, enrolled on 09/03/2019, did not contain the address for the second emergency contact listed. 2) The record for Child #2 did not contain the phone number for the two emergency contacts listed. 3) The record for Child #4, enrolled on 06/17/2019, did not contain the address for the first emergency contact listed. 4) The record for Child #5, enrolled on
09/09/2019, did not contain a name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the Center: The missing emergency contacts will be requested for each child. Management will develop a checklist for children's registration to ensure all the required information is listed for each child.
