Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today with the Assistant Manager and program staff from 6:55am to 8:30am. There were 25 children present, ranging from five to ten years of age, supervised by 2 staff. Only SHSI standards were reviewed during today's inspection. Five children's records were reviewed for allergy care plans. Two staff records were reviewed for documentation of current First Aid and CPR certification and the mandatory Virginia Preservice Training. The staff reported that no serious injuries have occurred. The most recent lock down drill was conducted on 10/10/19. The attendance and posted allergy list were reviewed. Standard #: 22VAC40-665-520-B-11
Description: Based on review of five children's records, the program did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The records for Child A, with a diagnosed allergy to nuts, and Child B, with a diagnosed allergy to pistachios and cashews, did not contain a written allergy care plan from the doctor.

Plan of Correction: I will get those from the parents as soon as possible.

Standard #: 22VAC40-665-580-E-1
Description: Based on review of on-site documentation for two staff, the program did not ensure that all staff had current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: The certificate for Staff A's CPR certification had a completion date of 10/18/17 with an expiration date of 10/18/19.

Plan of Correction: Staff A has already completed the first step of getting her CPR renewed, and she will see if she can complete the test today.
