An unannounced, mandated monitoring inspection was conducted today.
The inspector arrived at 3:55 PM and departed at 4:40 PM. Seven children were in care with two staff. After school activities and departures were observed. A sample size of five children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted to review the acknowledgement form prior to closure of the inspection.

Standard #: 22VAC40-185-140-A Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child has a physical examination before attending or within one month after attending.
Evidence:
1. The record for child 5 indicated that child 5 began attending on 9/7/2019. Documentation of a physical examination was not in the child's record.
2. Staff 1 verified that the record for child 5 did not include a physical examination.Plan of Correction: The facility responded with the following:
We will ensure that all children's records include documentation of a physical examination.
Standard #: 22VAC40-185-60-A Description: Based upon review of children's records and staff interview, the facility has not ensured that each child's record includes the street addresses of the two designated persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
1. The record for child 5 who began attending on 9/7/2018, did not include the street addresses for the two designated emergency contact persons.
3. Staff 1 verified that the street addresses for the emergency contact persons were not in the record of child 5.Plan of Correction: The facility responded with the following:
