Comments:
An unannounced monitoring inspection was conducted today between 3 and 6 pm. there were 86 children in care ages four years through nine years old with six staff. Children's records and staff emergency contact information was reviewed. Staff records are housed at a central location and will be reviewed at a later date. Children were observed in transitions and watching a movie. Violations regarding administration, staffing and supervision, programs and special care provisions and emergencies were identified and appear on the violation notice. The violations were reviewed with the site director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-350-C
Description: Based on observation and interview the center failed to ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group will apply to the entire group.
Evidence:
1 - Upon entering the program at 3:00pm staff 1 stated that there were 86 children present ages four years old in preschool to nine years old in elementary school.
a. The preschool children were observed being left as a part of the entire group while in the library.
2 - From approximately 3:35pm through 3:46pm there were 86 children present ages preschool (4 years old) through nine years old in the library with 3 staff.
a. Staff 2 left the room with 3 children identified as four year old preschool children. Leaving six preschool age sitting at table and unattended by the remaining two staff present in the room. Staff 2 stated that she was the preschool teacher.
a. One of the six staff remained seated near the entrance/exit of the program and was never observed as providing direct supervision to the large group of children. Therefore the number of staff providing direct supervision to the large group, at any one time was not greater than five.
b. All five staff were not consistently present in library until after, approximately, 4:35pm. From the time the children entered the library all staff were observed coming and going from the room at various times for various reasons.

Plan of Correction: Plan of Correction: Staff will be retrained on keeping preschool children in a separate group and maintaining a 10 to 1 ration.

Standard #: 22VAC40-185-360-A
Description: Based on observation the center failed to ensure that a variety of daily activities for all age groups shall be age and stage appropriate and provide opportunities for teacher-directed, self-directed, and self-chosen tasks and activities; a balance of active and quiet activities; individual and group activities; and curiosity and exploration.
Evidence:
1 - The only activity available to the children during the inspection was to watch a "Disney" movie.
a. Children ages four, in preschool through nine in elementary school sat waiting to watch the movie beginning at approximately 3:15pm. However, the movie did not begin until 3:56pm. There were two additional delays due to technical difficulties which required a non YMCA staff member's assistance to correct. During this time there was no effort to engage the children in any other activity, game or conversation.

Plan of Correction: Plan of Correction: Activity bags will be supplied to the site to use in the event the normal program space is not available.

Standard #: 22VAC40-185-430-K
Description: Based on observation and interview the center failed to ensure that provision are made for an individual place for each child's personal belongings.
Evidence:
1 - There was not an individual place for each child's belongings. Backpacks, lunch boxes and coats and jackets were haphazardly tossed on the floor of the library.
2 - Staff 1 confirmed that the basket normally used to store individual belongings was not present in the library.

Plan of Correction: Staff will be ensure baskets are available for children?s belongings

Standard #: 22VAC40-185-510-C
Description: Based on medication review and interview the center failed to ensure that a parent authorization is obtained with medication.
Evidence:
1 - There is and Epi Pen, for child 1, in the locked medication bag/box that does not have a parent authorization.
a. Child 1 was present and in care during the inspection today.
2 - Staff 1 confirmed that there is not a parent authorization for the Epi Pen.

Plan of Correction: Staff will obtain a parent authorization form for the Epi Pen.

Standard #: 22VAC40-185-540-A
Description: Based on observation and interview the center failed to ensure that each first aid kit is easily accessible to staff.
Evidence:
Staff 1 stated "the first aid kit is locked in the office. I can't go get it." When asked a second time staff 1 turned and walked away at approximately 5:04. Staff 1 returned at approximately 5:17pm.

Plan of Correction: : Staff will be retrained on maintaining a first aid kit in accessible locations.
