An unannounced renewal inspection was conducted today from 11:30am - 1:30pm. There were nine children in care ages three through five years old with two teachers and the program director. Children and staff records were reviewed. Board member and agent background checks were reviewed at the April inspection. A walking of the center was completed. Required postings, annual inspections, emergency evacuation drills first aid kit, medication and transportation were reviewed. Children were observed in teacher and self-directed activities including restroom routines, hand washing, lunch, and play activities and nap. Violations regarding children's physicals, physical plant, nutrition required postings were identified and appear on the violation notice. The violations were discussed with the program director on site at the conclusion of the inspection.

Standard #: 22VAC40-185-140-A Description: Based on record review and interview the center failed to ensure that each child has a physical examination by or under the direction of a physician within the allotted time frames.
Evidence:
1 - The record for child 1 does not contain any documentation of a physical examination. Child 1 has documented first date of attendance as 9/3/19.
2 - The record for child 2 contains a physical examination that is dated March 15, 2019 and a documented first date of attendance as October 25, 2019. Therefore, the information obtained within 12 prior to attendance.
3 - Staff 1 confirmed that there is no physical report in the record of child 1 and the physical obtained for child 2 was outside the allotted time frame.Plan of Correction: Physicals that meet the requirements will be requested from both parents.
Standard #: 22VAC40-185-280-B Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1 - There were several containers of Clorox wipes and a gallon container of liquid bleach on the floor of the closet in the two year old room.

b. The hazardous chemicals were within reach of children.

Standard #: 22VAC40-185-560-F Description: Based on observation and interview the center failed to ensure that when centers choose to provide meals and snacks that age-appropriate nutritional requirements of a recognized authority such as USDA are followed.
Evidence:
1 - The meal served to children consisted of four or five canned raviolis, mixed vegetables, apple/peach/mango sauce and strawberry milk.
a. one 14 ounce of mixed vegetables, one 40 ounce and one 15 ounce can of beef ravioli , six pouches of single serving fruit sauce was available for the nine children in care ranging age from three to five years old. The amount of protein, fruit and vegetable available did not meet the recognized requirements.
b. one percent flavored milk was served to all children. Flavored milk does not meet recognized requirements.
c. The fruit sauce served to children had an expiration date of September 15, 2019.
2 - Staff 2 was unable to explain how the food served did meet the recognized requirements.

Standard #: 22VAC40-80-120-E-2 Description: Based on observation and interview the center failed to ensure that findings of the most recent inspection of the facility are posted.
Evidence:
1 - The inspection results posted at the center were dated April 26, 2019. The most recent inspection was conducted July 12, 2019.
