Provided consultation on:
1) When centers choose to provide snacks, they must follow the most recent age-appropriate nutritional requirements of a recognized authority such as USDA;
2) Diapering area requirements, required disposal containers and leakproof, covered receptacles for soiled linens;
3) Safe sleep and crib linens

Comments:
An announced initial study was conducted from 9:30 am - 4:00 pm with the owners and center director. The classrooms were observed and measured for capacity. The center is well-equipped with age and stage appropriate furnishings, equipment, and materials. The cribs meet Consumer Product Safety Commission (CPSC) federal safety standards for cribs (manufactured 3/2019). Nine staff records were reviewed. The center's policies and procedures, emergency preparedness plan, and playground safety procedures were reviewed. Two playgrounds were observed. A qualified director is in place and was present during the inspection. The center has a current fire inspection, health inspection, and certificate of occupancy. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of nine staff records, not all staff had a Tuberculosis (TB) test/screening completed within 12 months prior to employment. Evidence: Staff #1 (DOH: 6/17/19) had a TB test/screening that was dated: 3/1/18.

Plan of Correction: Staff will obtain a new TB test/screening.

Standard #: 22VAC40-185-70-A
Description: Based on review of nine staff records, it was determined that not all records had all required information. Evidence: The records for staff #1, 2, 3, 4, 5, 6, 7, 8, and 9 did not have the name, address or phone number of a person to be notified in an emergency. Staff #2, 3, 4, 5, 6, 7, 8, and 9 did not have written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position.

Plan of Correction: Will ask staff to provide all missing documentation.

Standard #: 22VAC40-185-240-D-5
Description: Based on review of documentation, and staff interview, it was determined that there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Plan of Correction: Staff will obtain daily health observation training as soon as possible.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility did not have the required nonmedical emergency supplies. Evidence: there were no batteries for the battery operated radio or flashlight.

Plan of Correction: Will purchase batteries for the flashlight and radio.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of nine staff records, the facility did not have an Office of Background Investigations (OBI) fingerprinting-based criminal history record determination letter prior to a staff beginning employment. Evidence: Staff #1's (date of hire: 6/17/19) determination letter was dated: 10/2/19; Staff #2, #3, #4, and #5 (date of hire: 11/11/19) had a determination letter dated: 11/13/19; Staff #6, #7, #8 and #9 (date of hire: 11/11/19) did not have a determination letter in their record.

Plan of Correction: All staff have been fingerprinted and we are waiting on the results. In the future, staff will not start working until a fingerprint determination letter has been received.
