Comments:
A inspection was conducted on November 14th, 2019 in response to a complaint that was received by the licensing office on November 12th, 2019. After inspecting the facility, it was determined that the complaint allegation was valid. Non complaint related violations were cited on the Violation Notice. Complaint related: Yes
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition Unsafe conditions shall include, but not limited to splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment, head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
The elevator was determined to be inoperable.

Plan of Correction: I will have the elevator repaired.

Description: Based on observation, the center failed to ensure that each restroom area provided for children shall be equipped with soap, toilet paper and disposable towels or an air dryer.

Evidence:
The bathroom on the first floor was not equipped with soap.

Plan of Correction: Corrected during the inspection.

Complaint related: No
Description: Based on observation, the center failed to ensure that a nonabsorbent surface for diapering or changing shall be used.

Evidence:
The changing pad in the Jr, Toddler class is ripped and therefore is no longer nonabsorbent.

Plan of Correction: I will replace the changing pad.
