Reviewed 22VAC40-185-500-A.1 and 22VAC40-185-560-G.1 related to hand washing and food brought from home.

Comments:
An unannounced monitoring inspection was conducted at the facility on November 14, 2019 from approximately 9:35 am until 12:10 pm. Upon entrance there were 32 children present with 3 staff members and 7 parent volunteers. Additional staff arrived on site after inspection began for administrative and support purposes. The licensing inspector observed the children playing on the playground, preparing for a field trip, reading and eating snack. The sample size of records reviewed included 5 children's records, 4 staff records and 2 parent volunteer records. Two medications were also reviewed as a part of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence;
On the playground there was a plastic storage bin and there were two sharp screws protruding from the side of the bin. These screws were accessible to the children.

Plan of Correction: The front of the bin on the playground was immediately snapped back into place to cover the exposed screws. The chair of the playground committee was contacted and will assess the bins to determine a permanent fix that will ensure this will not happen again. Staff were instructed to monitor the bin daily to make sure the screws do not become exposed again. Date corrected ? November 14, 2019

Description: Based on observation and staff interviews, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence;
Hazardous substances (sanitizing wipes) were observed and were not kept in a locked place. One container of sanitizing wipes were stored on the mini refrigerator near the restrooms and one container was on a book shelf in the two year old room. The containers were labeled "keep out of each of children" and "caution":

Plan of Correction: The Clorox wipes were immediately placed in the locked cabinet where they belong. Staff were reminded that these wipes must be stored in the locked cabinet at all times. Date corrected ? November 14, 2019
