An unannounced monitoring inspection was conducted on 11/18/2019 from 4:10 pm - 5:15 pm. At the time of entrance there were ten children in care with two staff. The children ranged in age from four years - school age. Children were observed eating dinner, coloring pictures, playing games, reading and playing basketball in the gym. Records were reviewed for six children in care. Medication and emergency supplies additionally reviewed.
Two violations were observed and are listed on the violation notice.

Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence:
1. The enrollment record for child 1 did not contain documentation of a physical exam.
a. Child 1 has an enrollment date of 09/03/2019 and was in care during the inspection.
2. The program director confirmed that a documented physical was not on file for child 1.Plan of Correction: Documentation of a physical exam will be obtained for child #1.
Standard #: 22VAC40-185-510-E Description: Based on medication review, the center failed to ensure that medication shall be labeled with the child's name.
Evidence:
1. One container of over-the-counter medication was not labeled with the child's name.
a. The medication was stored inside of a labeled plastic bag however, the medication bottle/container was not labeled.Plan of Correction: The medication was labeled during the inspection.
