An unannounced renewal inspection was completed on November 21, 2019 from approximately 1:30 p.m. to 4:00 p.m. Twenty-eight children were in care and five staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include nap time, free play, and snack time. Interviews and interactions were conducted and there are no children currently taking medications. Five children?s records, five staff records and one corporate officer record was reviewed. The director/owner and assistant director were available for the inspection and were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-40-J Description: Based on a review of records and interview, the licensee failed to update the injury prevention procedures at least annually based on documentation of injuries and a review of the activities and services.
Evidence: 1. A review of the injury prevention plan revealed the last update was completed in August, 2017 and therefore not annually as required. 2. In an interview, Administrator #2 reported she's sure it has been completed but acknowledged being unable to locate it.Plan of Correction: The owner will locate the most recent injury prevention plan, which she believes is at her home office, and bring it to the center.
Standard #: 22VAC40-185-330-B Description: Based on observation of the playground and interview, the licensee failed to maintain resilient surfacing under equipment with moving parts or climbing apparatuses.
Evidence: 1. A multicolored spring rocker, with four seats approximately two feet off the ground, was observed located over asphalt. 2. In an interview with Administrator #1, she reported the children use this piece of equipment and that the center was unaware that it needs to be over resilient surfacing.Plan of Correction: Per the owner, this piece of equipment will not be used until mulch is ordered and delivered to go under it.
