Comments:
An unannounced monitoring inspection for the renewal of the program's license was conducted on 11/15/19 from. to 3:00 p.m. to 5:00 p.m.
At the time of entrance the following adult to child ratios were observed:
5 children, ages 4 to 5 years old, with one staff
33 children ages 6 to 8 years old with 3 staff

The records sample size consisted of 5 children?s records. Four (all) staff records for this site were reviewed at the Blocker YMCA administrative offices on 11/19/19.

Children were observed completing homework assignments, playing table top games and gross motor games in the gymnasium, and eating an afternoon snack which consisted of pretzels and fruit juice. Restroom procedures and table sanitizing were also observed. Medication administration was not observed. The center had four medications stored on site which were reviewed with regard to storage, labeling, written authorization and required documentation. Staff report that children are not routinely transported while in care and that there have been no accidents or injuries requiring first aid or medical attention since the last inspection. Violations were found during the inspection and are identified on the violation notice. Thank you for your time during today's inspection. Standard #: 22VAC40-185-510-A
Description: Based on a review of medication stored on site for children in care and written authorization forms, the program was not able to demonstrate that medication is administered to children only with written parent authorization.
Evidence:
The program identified two children in care as having a food allergy.
The written emergency plan that the program accepted for child #7 documented that child #7 had allergies to cashews and Pistachios.
The plan, completed and signed by the child's physician said that child #7 must be administered Benadryl and/or Epinephrine if an allergic exposure occurs.
The form was not signed by Child #7's parent and no other written authorization was available to administer Benadryl or Epinephrine.

Plan of Correction: Staff #1 stated that he would speak with child #7's parent at pick up the same day and have them review and sign the allergy plan authorizing the administration of the medications in an emergency allergic reaction.

Description: Based on a review of medication stored on site for children in care and written authorization forms, the program was not able to demonstrate that they have followed their center procedures to include methods to prevent use of outdated medication.
Evidence:
The program identified two children in care as having a food allergy.
The written emergency plan that the program accepted for child #6 documented that child #6 had allergies to eggs and tree nuts..
The plan, completed and signed by the child's physician and the child's parent said that child #6 must be administered Benadryl and/or Epinephrine if an allergic exposure occurs.
The medication labeled with child #6's name included Benadryl and/or Epinephrine.
This Benadry was labeled with an expiration date of 9/2019.

Plan of Correction: Staff #1 stated that he would speak with child #6's parent at pick up the same day and ask that the medication be replaced by Monday.
