Comments:
An unannounced monitoring inspection was conducted on Friday, November 8, 2019 to determine the center's compliance with licensing standards and to follow-up on previously cited violations. The inspector arrived at 11:40am and concluded the inspection at approximately 3:40pm. The center's director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 123 children in the direct care of 21 staff members. Additional staff were present and available to the children in care. Upon the inspector's arrival, the children were engaged in a variety of activities. In the infant classrooms, some staff were observed sitting on the floor with a group of infants providing individual care and gross movement activities. Other staff were rocking and feeding children. Diapering and hand-washing procedures were also observed in these rooms. The children in the toddler classroom were observed preparing for nap time. Staff were assisting the children with cleaning up and washing their hands. Other classrooms were observed singing with staff, reading stories, taking bathroom breaks, and washing hands in preparation for lunch. The annex building was also inspected and found to be in compliance. The school-age children were not present during the inspection. Both buildings were decorated with children's art and seasonal decorations. All areas of the facility were inspected. The facility has age appropriate materials and supplies. Two buses and all playgrounds were inspected and found in compliance. Staff interactions with children were positive.The inspector interacted with staff and children in care.

Ten children's records, five medications, and ten staff records were reviewed. The first aid kit, flashlight, and weather band ratio were observed and in compliance. The most recent evacuation drill was documented on 10/16/2019. The most recent shelter-in-place drill was documented on 09/05/2019. The center will have to complete one more shelter-in-place before the end of the year.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of ten staff records on 11/08/2019, the center did not ensure that two staff submitted documentation of a negative tuberculosis (TB) screening no later than 21 days after employment.

Evidence: 1) The record for Staff #3, hired on 03/25/2019, contained a tuberculosis screening that was dated 05/05/2019. 2) The record for Staff #4, hired on 04/23/2019, contained a tuberculosis screening that was dated 07/09/2019. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Plan of Correction: Per the Center: Moving forward, staff will be requested to provide TB results before they are hired. If not submitted timely, administration will place staff on unpaid suspension until TB results are returned.
